Assistance Information

United Ministries can help families in a financial crisis pay their past due rent, mortgage, or utility bills. We offer financial assistance as funds are available.

Priorities for Assistance

Our priorities for assistance are that you:

  • Are working and/or are able to support your household in the near future
  • Have a dependent child (typically 18 years old or younger) living with you
  • Provide verification of your recent crisis
  • Have not previously received assistance from United Ministries on three or more occasions

To apply for assistance:

  • Fill out our online qualification form to see if you qualify for assistance
  • Make sure you complete the FDIC Play Money Smart Course and get a copy of the Completion Certificate to upload to your application. The link to the required “Your Income and Expenses” course is: https://playmoneysmart.fdic.gov/games
  • The application will open each Sunday from 7am-7pm, and you can apply at that time; we strongly recommend that you gather the documents listed below so that you can fill out a complete application when it opens on Sundays

What to Upload in the system

Rent Required Documents

Identification: ID verification is required for every member of the household

  • Valid state ID or passport for every adult in the household
  • Social Security Card for every member of the household
  • Birth Certificate for every child in the household

Income Verification: Varies based on the situation. All income needs to be verified, see the example list below. You only need to submit what applies to your household. 

  • Paycheck Stubs (90 days worth)
  • Bank Statements – 90 days worth (Chime, Cash App, Venmo, Apple Pay, SoFi, etc…)
  • Savings Statements (90 days worth)
  • Food Stamp Statement
  • Child Support Order Statement
  • SSDI Statement (each qualifying family member)
  • SSI Statement (each qualifying family member)
  • TANF Statement
  • Unemployment Statement
  • Offer Letter for Employment (including # of hours, pay rate, and start date)
  • Letter of Termination
  • Other

Expense Verification: Varies depending on what bill you are requesting assistance with

For Rent Requests

  • Copy of 3-day eviction notice (if applicable)
  • Copy of your lease
  • Copy of your ledger
  • Property Manager or Landlord’s name, email address and phone number

Crisis Verification: We must verify the crisis that caused your household to fall behind on the bills. This varies greatly. Some of the common crises we see include hours cut at work, hours lost at work due to illness or a child’s illness, job loss, unexpected medical expenses, unexpected vehicle repair, paid expenses for the death of an immediate family member, etc…

  • Paid receipt for Vehicle Repair 
  • Medical Documentation
  • Paid receipt for Medical Bill(s)
  • Letter of child’s absence from school from doctor’s office
  • Paid receipt for Funeral Expenses
  • Death Certificate of immediate family member

We will need verification of whichever crisis applies to your situation. If you have recently been laid off and received no formal notice, we have an Employment Verification form that your former employer can complete. Please contact us at Client@United-Ministries.com, to request this form.

Mortgage Required Documents

For Mortgage-related assistance: If you are more than 1 month behind, contact Habitat for Humanity (Karl Gambrell at (850) 434-5456 x117) first. We need an actionable plan and an email from Karl that you have corresponded with him.
